Одной из причин, ухудшающей результаты герниопластики, является ожирение. Актуальность проблемы хирургической коррекции абдоминальных деформаций у пациентов с избыточной массой тела обусловлена не только большим количеством осложнений раннего и позднего послеоперационного периода, рецидивов, но и неопределенностью в тактико-методологических подходах. Так же, больные с морбидным ожирением имеют высокое интраабдоминальное давление по сравнению с пациентами без ожирении. Целью работы является улучшение результатов лечения и качества жизни пациентов с избыточной массой тела, страдающими срединными вентральными грыжами, путем сочетания герниопластики с реконструктивными вмешательствами на передней брюшной стенке. В основу работы положены изучение результатов хирургического лечения 73 пациентов с вентральными грыжами на фоне избыточной массой тела. Во всех случаях выполнялась ненатяжная герниопластика с использованием сетчатых эксплантов. В качестве сравнительных критериев течения раннего послеоперационного периода в двух группах пациентов были выбраны: степень выраженности болевого синдрома, сроки восстановления функции кишечника, длительность операций, наличие осложнений, оценки функционального состояния мышечного аппарата передней брюшной стенки, средние сроки пребывания в стационаре. Увеличение объема и времени проведения герниоабдомининопластики не приводит к росту частоты как ранних, так и поздних послеоперационных осложнений. Симультантная герниоабдоминопластика патогенетически обоснованное оперативное вмешательство у пациентов с избыточной массой тела и наличием абдоминоптоза, страдающих вентральными грыжами.
One of the reasons worsening the results of hernioplasty is obesity. The relevance of the problem of surgical correction of abdominal deformities in overweight patients is due not only to the large number of complications of the early and late postoperative periods and of relapses, but also to the uncertainty of tactical and methodological approaches. Patients with morbid obesity also have high intra-abdominal pressure compared with patients without obesity. The aim of the work is to improve the treatment results and the quality of life of overweight patients suffering from midline ventral hernias by combining hernioplasty with reconstructive interventions on the anterior abdominal wall. The work is based on the study of the results of surgical treatment of 73 patients with ventral hernias against the background of excess body weight. In all cases, tension-free hernioplasty using mesh explants was performed. The following were chosen as comparative criteria of the course of the early postoperative period in the two groups of patients: the severity of the pain syndrome, the time of restoration of intestinal function, the duration of the operations, the presence of complications, the assessment of the functional state of the muscular apparatus of the anterior abdominal wall, and the average length of hospital stay. An increase in the volume and duration of hernioabdominoplasty does not lead to an increase in the frequency of either early or late postoperative complications. Simultaneous hernioabdominoplasty is a pathogenetically justified surgical intervention in overweight patients with abdominoptosis suffering from ventral hernias.

Ikki guruh bemorlarda erta operatsiyadan keyingi davr kechishining qiyosiy mezonlari sifatida quyidagilar tanlandi: og‘riq sindromining ifodalanish darajasi, ichak faoliyatining tiklanish muddatlari, operatsiyalar davomiyligi, asoratlar mavjudligi, oldingi qorin devori mushak apparatining funksional holatini baholash, statsionarda bo‘lishning o‘rtacha muddatlari. Gernioabdominoplastika hajmi va o‘tkazish vaqtining ortishi na erta, na kech operatsiyadan keyingi asoratlar chastotasining oshishiga olib kelmaydi. Simultan gernioabdominoplastika ortiqcha tana vazni va abdominoptozi bo‘lgan, ventral churralar bilan og‘rigan bemorlarda patogenetik asoslangan jarrohlik aralashuvdir.
